Creatine is stored in muscle mostly as phosphocreatine. Muscle cells use it to rapidly regenerate ATP, the molecule that fuels short, intense effort (Kreider et al., 2025).
This matters most during activities like heavy lifting, a sprint, or a fast flight of stairs. ATP runs out quickly during this kind of effort, and phosphocreatine helps replace it.
The body also makes some creatine naturally and gets more from foods like red meat and fish. Supplementing raises muscle creatine stores beyond typical dietary levels (Kreider et al., 2025).
Creatine and Muscle After 40
Muscle mass and strength tend to decline with age, and these changes can accelerate during the postmenopausal years. This increases the risk of sarcopenia. It is a syndrome marked by loss of muscle strength, mass, and physical function (Naddafha et al., 2026).

The benefit depended on dose and training. Trials using at least 5 grams a day alongside resistance training showed measurable gains in lean mass and strength. Lower doses without training showed no measurable effect (Naddafha et al., 2026).
A small 14-week study looked at perimenopausal and postmenopausal women. It also reported improvements in lower-body strength with creatine and resistance training (Hall et al., 2025).
Effects on bone density, though, remain unclear. The same 2026 review found no significant change in bone density overall (Naddafha et al., 2026).
What About the Brain?
Creatine’s role in cognitive health is genuinely interesting, but it is not settled science. Research on creatine and cognition is still relatively young. Some trials suggest modest benefits for memory (Xu et al., 2024).
A 2024 meta-analysis found low-to-moderate certainty evidence for benefits in memory, attention, and processing speed. The authors called for larger, more rigorous trials (Xu et al., 2024).
European food safety regulators reviewed the evidence later that year and reached a more cautious conclusion. EFSA concluded that a cause-and-effect relationship had not been established. This applies specifically to creatine supplementation and improved cognitive function (EFSA NDA Panel, 2024).
In one small study of midlife women, creatine plus strength training improved sleep quality. It did not, however, produce a measurable cognitive change (Hall et al., 2025). Cognition remains a promising area, not an established benefit like strength.
Does Creatine Cause Weight Gain or Bloating?
An early uptick on the scale is common and expected. It usually reflects water pulled into muscle cells, not new body fat (Kreider et al., 2025).
